COVID-19 Testing

FOR MEAD SCHOOL DISTRICT STUDENTS/STAFF ONLY: Please contact your child's school to find out where and when testing is available each day and follow the information below.

Home Test Information
If testing yourself (staff member) or your child/student with a home test kit, please do the following:

  • If test is POSITIVE, notify your school.
  • The Mead School District continues to be compliance with state reporting requirements for both positive and negative test results. 

Testing at School
Below is the parental consent form and the test registration links for COVID-19 testing that the Mead School District can perform at school. If this is the first time you/student have been tested this year, you must complete both the parental consent form. If you have previously tested and registered this year, you/student do not need to complete the forms and registration again.

Parental Consent Form - This form is good for the 2022-23 school year. Parents/Guardians will be notified prior to any COVID-19 test. Bring completed form with you or email to school nurse if there is not one on file already.
