Prior to midnight before an in-person school day, parents need to "attest" to their child's health by completing the Daily Health Attestation Survey through an online service that will be sent to them via email or text at 3:00 p.m. Sunday through Thursday. Parents are required to submit the Daily Health Attestation Survey for the next in-person school day by midnight, then double-check for health changes in their child each morning. Students who do not have a health survey submitted will undergo a health screening upon arrival. If a student or staff member is diagnosed with COVID-19, the school should be notified immediately.
The survey will take less than one minute to complete for each child, each day. The first email or text will be sent each day at 3:00 p.m. If not completed, a reminder email will be sent out at 6:00 p.m. and a second reminder will be sent out at 9:00 p.m. The attestation questionnaire will close at 12:00 a.m. midnight each night. The questionnaire is available in five different languages, English, Arabic, Russian, Marshallese and Spanish.
Hard copies will be prepared for both Mead High School and Mt. Spokane High School zero-hour teachers (to get them started) and this will be the students' pass into class. Students may also show the proper attestation document that is emailed to them on a phone or Chromebook when they arrive. See below for a preview of the online survey.

To open the QR survey, push the camera button on your phone or tablet and center the QR code within the yellow square on the screen. A message will appear on your phone/tablet to ”Open “qualtrics.com” in Safari (iPhone) or your default browser. Click on the message to open the survey.
