FOR MEAD SCHOOL DISTRICT STUDENTS/STAFF ONLY: Please contact your child's school to find out where and when testing is available each day and follow the information below.
 
Below is the parental consent form and the test registration links for COVID-19 testing that the Mead School District can perform at school. If this is the first time you/student have been tested this year, you must complete both the parental consent form and the test registrationIf you have previously tested and registered, you/student do not need to complete the forms and registration again.

Parent Consent Form - This form is good for the 2021-22 school year. Parents/Guardians will be notified prior to any COVID-19 test. Bring completed form with you or email to school nurse if there is not one on file already.
 
Next, if this is your first time testing and registering, please click on the link and register only for the test you were directed to, which will either be Rapid 15-minute Antigen test or Curative PCR test (24-48 hour result).

Home Test Information

If testing yourself (staff member) or your child/student with a home test kit, please do the following:

  • If test is POSITIVE, notify your school.
  • If test is negative, please use the attestation form linked here, complete it and return it to your school via hard copy or email.
  • The Mead School District continues to be compliance with state reporting requirements for both positive and negative test results.
