Entry into the Transitions Program
Step 1 Arrange to meet with the Transitions Program Team, (465-6008). The student must submit a letter requesting an entrance into the program.
Step 2 Once this letter has been received, through the Mead Dictrict Student Service Office, the Transitions Program teacher will contact the parents/guardians to set up an entrance interview.
Step 3 Parent(s)/Guardian(s) and student attend the interview process. This interview is located in the Transitions Program classroom at Mead Support Service. Once accepted, the program’s teacher will arrange a time for the student to start the program.
Step 4 Once the student has completed the program, arrangements for the Exit Presentation Conference will be scheduled. This conderence includes a presentation of the student's project and an interview by the Transitions Exit Interview Committee .
Step 5 Upon successful completion of the Transitions Program exit interview, and with mutual constant, the student may return to school.
